Yeah, you pretty much have to use water with them.
Its basically transfers heat via electricity from one side (the cold plate) to the other (hot side)
You place it on your CPU cold plate down, then drop the waterblock on it. plug it in.. and go.
But they use a lot of enegry. Generally not regarded as anything special. Was a simple way to subambient temps back in the day though.
