Most of the time I get that problem it's because the card is tilted down a little in the back from fiddling with something and the front of the card lifts up out of the slot a tad and loses contact.

I thought of the tilted thingy because as you know the lightning with a fat boy is extremely heavy, and with board insulation, sometimes difficult to seat correctly.
but I ran startups with the air cooled card in every concievable position (well, except 69 of course
: ) Even held it at different angles to no avail.
